Medi-Cal Checklist

Required Documentation:

• Proof of Identification: California Driver’s License or California ID
• Social Security Card
• Most Recent Bank Statement (three months back, if applying for retro benefits).
• Automobile Registration (Department of Motor Vehicles)
• Verification of Health Insurance Premium Payment
• Green Card for Non-Citizens
• Verification of Cash Value on Whole Life Insurance Policies
• Verification of All Income Sources:
• State Disability Insurance (SDI) check stub or award letter
• Private Disability Insurance award letter, if applicable
• Social Security Notice of Award Letter (Current Award Amount)
• Recent Employer Check Stub, if applicable
